Dr. Conrad Allen Hazen, 86, of Ormond Beach, Florida passed away peacefully on Wednesday, December 4, 2019 at Carlton Shores Vista Suite, Daytona Beach. Born April 13, 1933, the youngest of nine children, in Pittsburgh, Pennsylvania to Frank and IdaMae (Male) Hazen. Dr. Hazen has been a resident of Volusia County since 1945 when the family moved from Pittsburgh. He attended North Ridgewood Elementary and graduated from Mainland High School where he played on the school's basketball team in 1952. Conrad served his country in the US Army during the Korean War in the Dental Corps. Upon his discharge from service, he attended Florida State University where he majored in Chemistry. On June 10, 1956, he married the love of his life, his wife of 55 years, Marilyn Moffett. He went on to attend Emory University School of Dentistry and graduated with a DDS in 1961. Dr. Hazen proudly practiced his trade in dentistry from 1961 ~ 1998. He attended Ormond Beach Baptist Church for 25 years and served as a Deacon, Sunday School Teacher and was a member of many committees. Currently, he had attended and served at the First United Methodist Church of Ormond Beach. Conrad was a member of the Daytona Beach Masonic Lodge # 81; 32 ^nd degree Scottish Rite; the Volusia County Dental Society, where he served as President; Central District Dental Society; Florida Dental Association; American Dental Association; was a member of the Xi Psi Phi Dental Fraternity;  Fellow of American Academy of Forensic Sciences and the Academy of Forensic Odontology. Dr. Hazen was a man of deep faith and conviction. He enjoyed his profession and patients and it gave him great satisfaction to make others feel good. His enjoyments included fishing, reading, working out and mostly his time with family. He is survived by his two sons, Jeffrey Paul Hazen and his wife Karie of Jacksonville Beach, Florida, and Brian Wade Hazen and his wife Michelle of Ormond Beach. by his three grandchildren, Casey Allen Hazen, Christopher Russell Hazen, and Karly Marie Hazen, a step-grandchild, Cassidy Crowell, as well as four sisters, Marjorie Burnsed, Cloyd Long, Jean Meyers, and Thelma Mitzel. He was preceded in death by his parents, his wife of 57 years, Marilyn, his brother, Frank Hazen, and three sisters, IdaMae Cooper, Katherine Bordo, and Dorothy Funeral Service will be celebrated on Saturday, December 7 ^th at 5:30 pm at Lohman Funeral Home; 733 W. Granada Blvd. Ormond Beach. The family will receive friends one hour prior to services. Interment will be held at a later date at Volusia Memorial Park, Ormond Beach.
